NIH FORMS-I Human Subject and Clinical Trials Information Worksheet

This fillable form is intended to help U-M research administrators collect the detailed data and scientific questions they will need from principal investigators to more readily complete FORMS-IH and the PHS Human Subjects and Clinical Trials Information Form in the eResearch Proposal Management (eRPM) system.

U-M eRA Commons Request Form

Use this form for requests to obtain a new eRA Commons ID or to add/remove roles to an existing Commons ID. eRA Commons is the National Institutes of Health’s electronic system required for managing NIH proposals and awards. U-M ORSP staff are the designated officials who register and update the NIH eRA Commons IDs for all U-M faculty, staff and students. U-M MFTRP Certification Form

NIH requires Malign Foreign Talent Recruitment Program (MFTRP) certifications with research performance progress reports (RPPRs) annually for senior/key personnel to ensure investigators are not part of an MFTRP, which is against federal and U-M policy. If individuals are not including updated Other Support documentation as part of the RPPR that incorporates the digital certification for this requirement, they can upload a certification statement using this form. Uniform Guidance Request Template/Tool

This tool (U-M login required) is meant to assist project teams when submitting an award change request (ACR) for Uniform Guidance (UG) Monitored Costs. Project teams can use this optional form to list UG expenses and then upload this to the ACR form to help ensure the requirements are met to avoid delays or required changes. Safe and Inclusive Work Environments Plan Template

Attachment 3B for Subrecipient Contacts

Attachment 3B is a Federal Demonstration Partnership (FDP) Clearinghouse form used to collect subrecipient information required for contact information and Federal Funding Accountability and Transparency Act reporting. Non-U-M Affiliate Financial Interests Disclosure Form

This form is for when non-U-M investigators on a research project under the oversight of the University of Michigan must disclose outside interests to U-M. Subrecipient Commitment Form

A completed Subrecipient Commitment Form is required when requesting a subcontract.
